Job Description
We are seeking a highly motivated and detail-oriented Clinical Coder to join our healthcare data management team in Reading. As a Clinical Coder, you will play a crucial role in accurately translating complex medical information from patient records into standardized codes (ICD-10 and OPCS-4). Your expertise will ensure the integrity and quality of our healthcare data, supporting essential analytics, reporting, and research initiatives. Key Responsibilities
- Accurately abstract and code clinical information from patient medical records using ICD-10 and OPCS-4 classification systems.
- Ensure all coding complies with national and local coding standards, guidelines, and policies.
- Review clinical documentation thoroughly to identify principal diagnoses, comorbidities, procedures, and other relevant data.
- Liaise effectively with clinicians, medical staff, and other departments to clarify documentation queries and resolve discrepancies.
- Maintain high standards of data quality and integrity through meticulous record-keeping and validation processes.
- Participate in regular coding audits, quality assurance activities, and continuous professional development.
- Contribute to the improvement of coding practices and data capture methodologies.
Required Skills
- Proven experience as a Clinical Coder, with strong knowledge of medical terminology, anatomy, and physiology.
- Demonstrable proficiency in ICD-10 and OPCS-4 coding classifications.
- Exceptional attention to detail and a high degree of accuracy in coding.
- Strong analytical and problem-solving skills to interpret complex clinical information.
- Excellent communication and interpersonal skills, capable of effective collaboration with medical professionals.
- Ability to work independently, manage workload efficiently, and meet deadlines.
Preferred Qualifications
- National Clinical Coding Qualification (NCCQ) or an equivalent certification.
- Experience working with various Electronic Patient Record (EPR) systems.
- A degree or diploma in Health Information Management, Health Sciences, or a related field.
- Previous experience in a large acute NHS trust or private healthcare setting.
- Familiarity with clinical audit processes and data validation.
