Job Description

We are seeking a compassionate and dedicated Public Health Nurse to join our team. In this vital role, you will work directly within the community to assess health needs, provide essential health education, prevent disease, and promote wellness. This position requires strong clinical judgment, excellent communication skills, and a passion for community health.

Key Responsibilities

  • Conduct community health assessments and identify health disparities.
  • Develop and implement health education programs for various age groups and populations.
  • Provide direct nursing care, including immunizations, screenings, and health counseling, in clinics and community settings.
  • Investigate communicable diseases and implement control measures.
  • Provide home visits to clients for health monitoring, education, and support.
  • Collaborate with community organizations, schools, and healthcare providers to address public health priorities.
  • Maintain accurate and confidential client records.
  • Participate in emergency preparedness and response activities.
  • Advocate for policies that promote public health.
  • Monitor and evaluate the effectiveness of public health interventions.

Required Skills

  • Current Registered Nurse (RN) license in New Hampshire.
  • Strong clinical assessment and critical thinking skills.
  • Excellent verbal and written communication abilities.
  • Ability to work independently and as part of a multidisciplinary team.
  • Cultural competence and sensitivity to diverse populations.
  • Proficiency in basic computer applications and electronic health records.
  • Valid driver's license and access to reliable transportation.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) degree.
  • Previous experience in public health, community health, or home health setting.
  • Bilingual proficiency (especially Spanish).
  • Certification in Public Health Nursing (CPHN).
  • Experience with health promotion program development and evaluation.
