Job Description
We are seeking a highly skilled and compassionate Clinical Pharmacist to join our dedicated team in Mount Pearl, Newfoundland and Labrador. Key Responsibilities
- Provide advanced clinical pharmacy services, including medication therapy management (MTM), disease state management, and pharmacotherapy consultations.
- Review, interpret, and dispense prescriptions accurately and safely, adhering to all provincial and federal regulations.
- Counsel patients on proper medication use, potential side effects, drug interactions, and adherence strategies.
- Collaborate with physicians and other healthcare professionals to optimize patient care plans and ensure continuity of care.
- Conduct medication reconciliation and identify opportunities for medication optimization to improve patient outcomes.
- Participate in health promotion and disease prevention initiatives within the pharmacy and local community.
- Manage and oversee pharmacy technicians and support staff, ensuring efficient workflow and high standards of practice.
- Maintain accurate patient records and ensure compliance with all privacy and confidentiality standards.
- Stay current with pharmaceutical advancements, clinical guidelines, and best practices through continuous professional development.
Required Skills
- Doctor of Pharmacy (Pharm.D.) or Bachelor of Science in Pharmacy (BSc.Pharm).
- Active registration and good standing with a pharmacy regulatory body in Canada or eligibility for licensure in Newfoundland and Labrador.
- Demonstrated clinical knowledge and experience in a community or hospital pharmacy setting.
- Excellent communication, interpersonal, and patient counseling skills.
- Proficiency in pharmacy software systems and basic computer applications.
- Strong problem-solving abilities and attention to detail.
- Ability to work effectively in a fast-paced, collaborative team environment.
- Commitment to patient safety and evidence-based practice.
Preferred Qualifications
- Advanced clinical certifications (e.g., CDE, BCPS).
- Experience with medication therapy management (MTM) services.
- Familiarity with the Canadian healthcare system and provincial pharmacy regulations.
- Previous experience supervising pharmacy support staff.
- Proficiency in additional languages.
