Job Description

We are seeking a dedicated and compassionate Case Manager (Social Work) to join our Rockhampton team. In this vital role, you will provide comprehensive case management services to individuals and families facing complex challenges, ensuring they receive the support and resources needed to achieve their goals. You will work within a supportive government and not-for-profit framework, empowering clients to navigate social, emotional, and practical barriers, and advocating for their best interests. This role offers the opportunity to make a tangible impact within the local community.

Key Responsibilities

  • Conduct comprehensive assessments of client needs, strengths, and goals.
  • Develop, implement, and monitor individualised case plans in collaboration with clients.
  • Provide crisis intervention and support as required, linking clients to appropriate emergency services.
  • Advocate on behalf of clients to access housing, health, legal, and other essential services.
  • Maintain accurate and confidential client records and documentation.
  • Collaborate with internal teams and external agencies to ensure holistic client support.
  • Facilitate group programs and workshops to enhance client skills and well-being.
  • Participate in supervision, team meetings, and professional development activities.

Required Skills

  • Bachelor's degree in Social Work or a related field (e.g., Human Services, Psychology).
  • Demonstrated understanding of case management principles and practice within a social work framework.
  • Strong interpersonal, communication, and active listening skills.
  • Ability to work effectively with diverse populations, including those from Aboriginal and Torres Strait Islander communities.
  • Excellent organisational and time management skills, with the ability to manage a caseload effectively.
  • Proficiency in maintaining accurate and confidential client records.
  • Current Queensland Driver's Licence and access to a reliable vehicle.

Preferred Qualifications

  • Post-graduate qualifications in Social Work or related discipline.
  • Experience working within government or not-for-profit sectors.
  • Knowledge of local community resources and support networks in Rockhampton.
  • First Aid and CPR certification.
