Job Description

We are seeking a dedicated and organized Hospice Intake Coordinator to join our compassionate team in Braunschweig. This vital administrative role involves managing the initial contact with prospective patients and their families, guiding them through the intake process for our hospice services. The ideal candidate will possess excellent communication skills, a high degree of empathy, and strong organizational abilities to ensure a smooth and supportive experience for those seeking our care. You will be the first point of contact, making a significant impact on families during a sensitive time.

Key Responsibilities

  • Serve as the primary point of contact for all new hospice inquiries, responding promptly and empathetically via phone, email, and in-person.
  • Conduct initial assessments to gather necessary patient information, eligibility criteria, and service needs.
  • Explain hospice services, admission criteria, and available resources clearly and comprehensively to patients and families.
  • Coordinate with clinical staff, social workers, and other team members to facilitate seamless patient transitions.
  • Maintain accurate and confidential patient records in accordance with organizational policies and data protection regulations (e.g., GDPR).
  • Process all required intake documentation, ensuring completeness and compliance.
  • Provide emotional support and resources to families navigating end-of-life care decisions.
  • Assist with scheduling initial home visits or consultations.
  • Collaborate with external healthcare providers and referral sources to streamline the intake process.

Required Skills

  • Proven experience in an administrative or coordination role, preferably within healthcare.
  • Exceptional interpersonal and communication skills, both written and verbal.
  • High level of empathy, compassion, and sensitivity when interacting with patients and families.
  • Strong organizational skills and attention to detail.
  • Proficiency in office software (e.g., Microsoft Office Suite, patient management systems).
  • Ability to work independently and as part of a multidisciplinary team.
  • Fluent in German and English.
  • Understanding of patient confidentiality and data protection regulations.

Preferred Qualifications

  • Experience specifically within hospice, home health, or palliative care settings.
  • Formal qualification in healthcare administration, social work, or a related field.
  • Knowledge of local healthcare resources and community services in Braunschweig.
